1. Cover letter explaining the following:
a. What you are studying.
b. The degree you are pursing.
c. Where.
d. Expected completion.
e. Any and all sources of funding you have.
2. A letter of support from your chair supporting your request.
3. A transcript.
4. Documentation that YOU have paid the costs that you are asking requesting be reimbursed.
